Short answer
Orforglipron is an Eli Lilly GLP-1 receptor agonist taken by mouth. Unlike Rybelsus (oral semaglutide, a peptide that needs fasting conditions for absorption), orforglipron is a small, non-peptide molecule. Phase 3 programmes (ACHIEVE in type 2 diabetes, ATTAIN in obesity) reported weight and glucose changes with the familiar GLP-1 gastrointestinal side-effect class — nausea, constipation, diarrhoea, vomiting. The FDA approved it in 2026 as Foundayo for chronic weight management; unlike oral semaglutide it has no food or water timing restrictions. It is not a peptide, and this site has no tagged confessions for it yet. Peptide Confessions does not publish dosing, timing, injection or sourcing guidance.
Approval and trial results
The FDA approved Foundayo on April 1, 2026, 50 days after filing, under its National Priority Voucher programme, for adults with obesity, or overweight with a weight-related condition. Last reviewed October 15, 2026.
| ATTAIN-1 (72 weeks, no diabetes) | Foundayo, highest dose | Placebo |
|---|---|---|
| Average weight change, all participants | About -11% | About -2% |
| Average weight change, if taken as directed | -12.4% | -0.9% |
| Nausea | About 34% | About 10% |
| Constipation | About 25% | About 9% |
| Vomiting | About 24% | About 4% |
| Stopped because of side effects | About 10% | About 3% |
How Foundayo differs from other GLP-1s
It is a small molecule rather than a peptide, so it can be taken any time of day with or without food. Average weight loss in its trials was lower than with injectable tirzepatide or semaglutide or the Wegovy pill, though no head-to-head trial has been published. Label warnings include pancreatitis, severe GI reactions, kidney injury from dehydration, gallbladder disease, low blood sugar with some diabetes drugs, and aspiration under anaesthesia; it shouldn't be combined with another GLP-1. Comparisons: GLP-1 pills, Foundayo vs Wegovy pill, Foundayo side effects.

Community Q&A
- When was Foundayo approved?
- April 1, 2026, by the FDA, for chronic weight management in eligible adults.
- How much weight do people lose on Foundayo?
- In ATTAIN-1, about 11% on average at the highest dose over 72 weeks (12.4% among those who stayed on it) vs about 2% on placebo.
- Do you need to fast before taking Foundayo?
- No. Unlike oral semaglutide, it can be taken any time of day with or without food or water.
- Is orforglipron approved?
- Yes. The FDA approved orforglipron as Foundayo in 2026 for weight management in eligible adults.
- Is orforglipron a peptide?
- No. It is a small-molecule GLP-1 receptor agonist, which is why it can be taken as a pill without peptide-style absorption limits.
- What are orforglipron side effects?
- Trials report the GLP-1 class pattern: mainly nausea, constipation, diarrhoea and vomiting, most often when starting or stepping up.
- Does this page give dosing advice?

- Is Foundayo a new drug?
- Yes. Foundayo is the brand name for orforglipron, Eli Lilly's small-molecule oral GLP-1 drug, which is not a peptide.
