Tesamorelin Benefits: What the Label Says and What Users Report

Tesamorelin benefits: what trials found for belly fat, liver fat, triglycerides, muscle and memory, what isn't proven, and what anonymous users report. Not medical advice.

Short answer

Tesamorelin is a GHRH analogue: it prompts the pituitary to release more of your own growth hormone, which raises IGF-1. In the US it is FDA-approved (brand Egrifta) to reduce excess abdominal fat in adults with HIV-associated lipodystrophy. The best-proven benefit is less visceral fat, the deep fat around the organs, along with lower triglycerides; smaller trials add liver fat. Wider claims (general fat loss, anti-ageing, muscle, skin, sleep) come from off-label and community use and are not what the trials tested. Peptide Confessions does not publish dosing, timing, injection or sourcing guidance.

Benefits tested in human trials

Doses are deliberately left out. Most participants had HIV, and most were men. Last reviewed October 19, 2026.

BenefitWho was studiedWhat the trial foundSource
Less visceral (deep belly) fat806 adults with HIV and excess belly fatVisceral fat about 15% lower than placebo at 26 weeks; fat under the skin barely changed; less distress about belly appearanceFalutz 2010
Lower triglyceridesSame trialsTriglycerides fell; responders (visceral fat down at least 8%) had better triglycerides, glucose and HbA1c than non-respondersFalutz 2007, Stanley 2012
Less liver fat61 adults with HIV and fatty liver, 12 monthsLiver fat fraction down 4.1 points more than placebo (a 37% relative drop); 35% vs 4% got below the fatty-liver cut-offStanley 2019
Belly fat without HIV60 abdominally obese adults with low growth hormone, 12 monthsLess visceral fat, lower triglycerides and CRP, slightly thinner carotid artery wall; no change in glucoseMakimura 2012
Trunk muscleResponders from the phase 3 trials (exploratory)Trunk muscle area and density rose a little vs placeboAdrian 2019
Memory and thinking152 adults aged 55 to 87 without HIV, 20 weeksA favourable effect on executive function; body fat down 7.4%Baker 2012
Memory and thinking in HIV73 adults with HIV and abdominal obesity, 6 months, open-labelWaist 2.7 cm smaller than standard care, but no significant cognitive differenceEllis 2025

What is not established

  • —Weight loss. The Egrifta WR label calls it weight neutral and says it is not indicated for weight loss management.
  • —Lasting results. In the trials, visceral fat came back after people switched to placebo (Falutz 2008).
  • —Muscle building, anti-ageing, skin, sleep or recovery in healthy people. No controlled trial has tested these.
  • —Long-term heart and blood vessel safety, which the label says has not been established. The label also notes that the effects of prolonged IGF-1 elevation are unknown.
  • —Benefits for women specifically, since 84% to 86% of phase 3 participants were men; see tesamorelin for women.

Benefits vs risks

The same growth hormone signal behind the benefits drives the main risks: fluid retention, joint pain, higher blood sugar and a higher IGF-1, plus allergic reactions. The label tells prescribers to weigh continuing in people whose visceral fat has not come down. Figures are on tesamorelin side effects.

What users report here

Anonymous accounts are collected on the Tesamorelin reports. They are personal stories, often with other compounds in the mix, not evidence of effect. The ones that describe a benefit mostly mention a smaller waist with the scale unchanged (#295, #81), which matches what the trials measured. Others describe things no trial studied, such as tighter skin and sharper night vision (#211) or relief from long-standing anxiety (#356). For the trial timeline at 26 and 52 weeks and what users describe, see tesamorelin results and before and after.

Community Q&A

What is tesamorelin approved for?
In the US, reducing excess abdominal (visceral) fat in adults with HIV-associated lipodystrophy. Other uses are off-label.
Does tesamorelin work for fat loss?
It reduced visceral fat by about 15% vs placebo in the phase 3 trials, but the label calls it weight neutral. Results outside that group are anecdotal; reports here are mixed.
Does tesamorelin help fatty liver?
In a 12-month trial of 61 adults with HIV and fatty liver, liver fat fell more than on placebo. It is not approved for fatty liver.
Does tesamorelin build muscle?
An exploratory analysis found a small rise in trunk muscle area in trial responders. It has not been shown to build muscle in healthy people.
Does Peptide Confessions give dosing advice?
Peptide Confessions does not publish dosing, timing, injection or sourcing guidance.
