Short answer

Tesamorelin (brand Egrifta) is a GHRH analogue that the FDA approved to reduce excess abdominal fat in adults with HIV-associated lipodystrophy. In its two phase 3 trials, led from the Montreal General Hospital at McGill, it cut visceral fat (the deep belly fat around the organs) by about 15% more than placebo over 26 weeks, while the pinchable fat under the skin barely changed. The label calls it weight neutral and says it is not indicated for weight loss. So a realistic tesamorelin before and after is a smaller waist, not a lower number on the scale, and in the trials the visceral fat came back after people stopped. Peptide Confessions does not publish dosing, timing, injection or sourcing guidance.

Timeline: what the trials measured

These are the main human studies, all in adults with HIV and excess belly fat; results in other people are not established. Doses are deliberately left out. Last reviewed October 18, 2026.

Not everyone responds

The trials counted someone as a responder if their visceral fat fell by at least 8%. Over 52 weeks, responders had better triglycerides and steadier blood sugar and HbA1c than non-responders (Stanley 2012). The label tells prescribers to weigh whether to continue in people whose visceral fat has not come down, to monitor IGF-1 and to check glucose before and during treatment; HbA1c reached the diabetes range in 5% on tesamorelin vs 1% on placebo by week 26.

What tesamorelin users report here

The tesamorelin hub has 30 anonymous reports; many are stacks, questions or side effects rather than results. The ones that describe a before and after line up with the trials in one way: the waist, not the scale. #295 is eight weeks in with the scale unmoved and two inches off the waist, and says to use a tape measure. #81 describes almost the same result in nearly the same words. Inches here are self-measured.

Others are slower or messier. #211 says the belly-fat part has been slow but noticed tighter skin and sharper night vision, which no trial studied. #387 says their stomach looked worse before it looked better. #451 combined it with retatrutide, lost six pounds in two weeks, stalled for three, and later suspected water retention from tesamorelin and creatine; fluid retention is a labelled effect. #591 claims a first-ever six-pack after eight weeks with no gym or diet change, and #658 describes a body recomposition with ipamorelin. Those go beyond what the trials found, since fat under the skin did not change significantly.

Some reports are about stopping. #107 describes an anaphylactic reaction at the first injection that needed emergency treatment. #658 gave it up after an allergic reaction. #380 had whole-body tingling, no solid food for three days and vomiting in week one, and quit. The label reports hypersensitivity reactions in 4% of trial patients and injection-site reactions in 25% vs 14% on placebo. #356 says years of anxiety vanished within a month, while noting they found no research to explain it. These are individual stories, not result rates.

Why tesamorelin before-and-after photos mislead

Visceral fat sits behind the abdominal wall. The trials measured it with CT scans; a photo cannot, and a tape measure is only a rough stand-in. Bloating and water retention can move a waistline either way. Several people in our archive run tesamorelin alongside a GLP-1 such as retatrutide or ask about pairing it with semaglutide, so a dramatic photo usually reflects the whole stack, diet and training. Clinics and sellers pick their best cases. If a photo shows large overall weight loss, tesamorelin is unlikely to be the main reason. See peptide before and after and the sibling page on sermorelin before and after.

Realistic expectations

Tesamorelin works through the pituitary, so the label rules it out in people whose hypothalamic-pituitary axis is disrupted, and in active cancer and pregnancy. Its approval covers HIV-associated lipodystrophy only; other use is off-label, and long-term cardiovascular safety has not been established. A prescriber would judge it by waist or visceral fat over months, alongside IGF-1 and glucose, not by photos. WADA prohibits GHRH analogues, including tesamorelin, for tested athletes. Side effects are covered on tesamorelin side effects, and how it compares with other GHRH peptides on tesamorelin vs sermorelin.

Community Q&A

How long does tesamorelin take to show results?
In the trials, IGF-1 rose within weeks and visceral fat was about 15% lower than on placebo at 26 weeks; at 52 weeks waist size was 3.4 cm smaller in people who kept going. That is a trial average, not a promise for any individual.
Does tesamorelin cause weight loss?
Not by design. The label calls it weight neutral and says it is not indicated for weight loss. It targets visceral (deep belly) fat, so the scale may not move.
Does tesamorelin reduce belly fat?
It reduced visceral fat in adults with HIV-associated lipodystrophy. The fat under the skin of the abdomen did not change significantly in the pooled trials.
What happens when you stop tesamorelin?
In the trials, visceral fat came back after people were switched to placebo; the benefit did not last beyond treatment.
What do tesamorelin users report?
On this site, a smaller waist with an unchanged scale, slow or uneven change, and some allergic reactions. They are anonymous stories, not result rates.
Does Peptide Confessions give tesamorelin dosing?
Peptide Confessions does not publish dosing, timing, injection or sourcing guidance. Not medical advice.
