Leptin: The Fullness Hormone and Leptin Resistance

Leptin explained: the fat-cell hormone that signals energy stores, leptin resistance in obesity, the approved drug metreleptin, and how GLP-1s differ. Not medical advice.

Short answer

Leptin is a 146-amino-acid hormone made by fat cells. Its level rises with body fat and tells the brain how much energy is stored. People born unable to make leptin have extreme hunger and obesity, which leptin replacement reverses. Most people with obesity, however, already have high leptin; their brains respond less to it, which is called leptin resistance. That is why leptin injections did not work as a general weight-loss drug.

Approved use and related drugs

Metreleptin (Myalept), a form of leptin, was FDA approved in 2014 for complications of leptin deficiency in generalised lipodystrophy. Its boxed warning covers neutralising antibodies and lymphoma, and it is only available through a restricted programme. Leptin is different from GLP-1, the gut hormone mimicked by semaglutide and tirzepatide, and from ghrelin, the stomach's hunger hormone (see ghrelin vs leptin on that page). Diet changes marketed to "fix leptin resistance" lack strong evidence. Last reviewed: October 2026.

Leptin tests and leptin-resistance claims

Leptin blood tests are used in specialist care, mainly for suspected leptin deficiency or lipodystrophy; in typical obesity a high result just reflects more body fat. GLP-1 drugs lower leptin as fat is lost. "Leptin" supplements do not contain leptin. Related: GLP-1 supplements.

Community Q&A

Do leptin supplements work?
They contain no leptin, and no supplement has been shown to reverse leptin resistance.
Should I get a leptin test?
It is mainly useful in specialist care for suspected leptin deficiency or lipodystrophy.
What does leptin do?
It signals stored energy to the brain, reducing hunger when fat stores are adequate.
What is leptin resistance?
When the brain responds weakly to leptin despite high levels, common in obesity.
Can you take leptin for weight loss?
Metreleptin is approved only for leptin deficiency in lipodystrophy; it was not effective for typical obesity.
