Canada \u00b7 Quebec coverage

Ozempic in Quebec: what RAMQ actually covers.

The short answer: RAMQ covers Ozempic as an exception drug for type 2 diabetes under specific conditions, and never for weight loss. Wegovy is not on the RAMQ list. Below is what Quebec's own documents say, with links so you can check them.

The RAMQ criteria for Ozempic

INESSS's listing conditions set the indication recognized for payment:

  • Type 2 diabetes, in combination with metformin, and
  • A sulfonylurea is contraindicated, not tolerated or ineffective.
  • The initial authorization lasts up to 12 months; renewal requires evidence that the patient reached their individual HbA1c target.

The prescriber files RAMQ's form 8212; patients can't apply themselves. What you pay still depends on whether you are in the public plan or a private group plan, plus the plan's deductible and coinsurance.

Why weight loss isn't covered

Quebec's public plan excludes drugs prescribed to treat obesity, along with cosmetic uses. For Wegovy, INESSS looked at a narrower use, improving cardiometabolic outcomes in adults with a BMI of 35 or more and weight-related conditions, and in February 2026 recommended refusal because it did not recognize the therapeutic value for that use. Private plans and other options

  • Quebec residents with access to a group plan must generally be covered by it; those plans must include at least the RAMQ list and may cover more, including Wegovy or Ozempic for other uses.
  • Ask your pharmacist whether a generic semaglutide is listed or substituted.
  • “Research” semaglutide sold online isn't covered by any plan because it isn't an authorized drug: see Health Canada's April 2026 advisory.

Frequently asked questions

Is Ozempic covered by RAMQ?
Yes, for some people with type 2 diabetes. Ozempic is a RAMQ exception drug (médicament d'exception): it is paid for people with type 2 diabetes, together with metformin, when a sulfonylurea is contraindicated, not tolerated or ineffective. The prescriber must request authorization.
Does RAMQ cover Ozempic for weight loss?
No. Quebec's public drug plan does not cover medications prescribed to treat obesity, and Ozempic's recognized indication for payment is type 2 diabetes only.
Is Wegovy covered in Quebec?
No. In February 2026, INESSS recommended against listing Wegovy for improving cardiometabolic outcomes in people with a BMI of 35 or more, because its therapeutic value for that use was not recognized. Some private plans may cover it.
How long does RAMQ authorization for Ozempic last?
According to INESSS's listing conditions, the initial authorization lasts up to 12 months. To continue, the prescriber must show the patient reached their individual HbA1c target.
Do private plans in Quebec cover Ozempic?
Group insurance plans in Quebec must cover at least the drugs on the RAMQ list, often under the same conditions, but many cover more. Check your plan booklet or ask your insurer.
Does this page recommend a clinic, product or dose?
No. We don't list clinics, sellers, prices or doses, and nothing here is medical advice. Talk to a Quebec prescriber or pharmacist.
